Falling caused a previous surgical apparatus in the hip to loosen, and Carolyn was soon met with other complications, a rollercoaster of pain and progress.

Background

Residing in our Assisted Living Facility, Carolyn experienced a fall in August of 2022 which resulted in the loosening of a previous surgical apparatus in her hip. After hospitalization, she was transferred to our Rehabilitation facility to receive therapy with the goal of returning to her Assisted Living apartment. When Carolyn arrived at Friendship Village, she was totally dependent for ambulation, transfers, and activities of daily. We began therapy immediately upon arrival and set short term and long term goals for Carolyn to regain her function one step at a time. However, she suffered an infection in that same hip from surgical complications which sent her back to the hospital for a lengthy stay. It was a roller coaster experience with her pain and overall progress after returning to Friendship Village.

Plan of Care

In January of 2023, Carolyn’s sons met with our team and new functional goals were set with a dynamic action plan to get her home as soon as possible. Our team started each day in Carolyn’s room to motivate and prepare her for treatment. We completed several home visits and made corrections as well as additions to her apartment to ensure a safe transition. By the time of discharge, Carolyn was able to ambulate over 100 feet with a walker, transferring on her own and completing all her activities of daily living without assistance.

Outcome

On March 27th, Carolyn was discharged home to her apartment independently.

It was a long road, but I’m excited to go back home. I couldn’t have done it without the therapy team.
