Our Story
Located just 20 minutes from beautiful Madison, Wisconsin, we at Heartland Country Village in the quaint town of Black Earth pride ourselves in being a smaller facility that provides our residents that special ‘at-home’ feeling. Established in 1975, we feel we deliver a more personal, yet higher quality of care when compared to larger, big-city facilities. Black Earth is a small community where everyone knows everyone, and everyone cares for everyone, so we feel our name ‘Heartland’ is very fitting for this loved area. The community describes Heartland Country Village as ‘family’, and we’re honored to serve every one of our 40 residents with the most compassionate care.
With a highly skilled team of 50 employees, each resident gets personal attention at Heartland Country Village. We provide multiple levels of services, including Physical, Occupational and Speech Therapy, as well as Intravenous Therapy (IV), Enteral and Total Parenteral Nutrition and Oxygen and Respiratory Therapy. We’re also a Long-Term Care facility, which includes Palliative and Hospice Care options. Heartland Country Village serves each resident’s individual needs so they can live happy, fulfilling lives.
We feel the best part of Heartland Country Village is our residents. They are the ‘Heart’ of Heartland. We have residents that have been with us for over 10 years and it’s our privilege to care for every single person as family. Our operational vision focuses on people, purpose, care and community.
